Policies and Procedures for Provenance:

Please ensure you read and understand the following information before arriving to your scheduled appointment.

We strive to establish and maintain the trust of people we serve. This requires clear communication between our team members and our community. We invite inquiry and would be pleased to clarify the measures we have implemented to ensure the safety and comfort of everyone.

General Information

Clinic hours are Monday to Friday 9:00am to 7:00pm
Saturdays 10am to 4:00pm
Sunday CLOSED

  • Please arrive alone for your scheduled appointment. Support persons will be asked to wait outside the clinic for the duration of your treatment. If a support person requires to be inside the clinic, they are welcome to be in our waiting area provided they are symptom free and healthy.
  • Intake forms and consent forms must be completed prior to arriving for your appointment. Please note that some consents are given by clients only after having treatment related discussions with their practitioner. You will be informed about this if this applies to you before your appointment.
  • Please familiarize yourself with our Cancellation Policy.
  • Wearing a mask in public indoor settings is not required by public health currently. The Provincial Health Officer (PHO) recommends the use of medical masks worn by both RMTs and patients when there is close, in person patient/therapist contact. We encourage open and safe conversations in our space regarding comfort of each individual regarding mask wearing.

Doing Our Part

  • Our team members will be doing a daily self assessment prior to every shift. Anyone who is feeling unwell will be required to stay home. We will not provide in-person care if we are sick or show signs of illness.
  • As a clinic policy, all practitioners will make discretionary calls regarding the wearing of masks for the duration of the treatment session. Your comfort and security is of utmost importance, therefore a conversation before your appointment to set out expectations is welcomed and encouraged. Additional Personal Protective Equipment (PPE) may be worn in accordance with the guidelines set out by the respective regulatory authorities.
  • Practitioners will maintain regular hand washing protocols, including upon entering the clinic, pre-treatment and post-treatment or after contacting any personal or patient belongings. Sinks are present in each of the treatment rooms for facilitating this necessity.
  • Treatment rooms are cleaned and sanitized during the 15 minutes between scheduled appointments.
  • Common areas will be cleaned and sanitized throughout the day.

How You Can Help

  • If you are exhibiting symptoms of any illness, please stay home.
  • Before your scheduled appointment, you will be asked to complete the COVID-19/ Communicable Diseases screening form which will be emailed to you 24 hours prior. Please DO NOT forget to complete this, as we do reserve the right to cancel your appointment if screening has not been completed.
  • If you arrive at your appointment and display symptoms of illness, we reserve the right to reschedule your appointment.
  • Please arrive at your allocated time. We will call you as soon as we are ready to receive you in the clinic. You will be taken directly into your treatment room.
  • Until we are able to relax our COVID-19 policies, please wait in your car for your appointment as we are still implementing social distancing practices in our clinic.
  • Keys are available to shared washroom facilities.
  • You will be asked to wash your hands before and after your treatment. Sinks are present in all treatment rooms. Hand sanitizer is also available for use.

PLEASE NOTE: patients who cancel their appointment due to experiencing symptoms of COVID-19 will be unable to book another appointment until the amount of self-isolation time mandated by the provincial health office has elapsed.
